A practitioner and their patient work from one shared list of goals. This page shows each of them how to do the things they need to do.
Three ways, fastest first.
Goals save as you add them. There is no separate save step, and the patient sees each one straight away.
Typing something close to a goal already on the list brings up a short prompt — use the existing one, add yours anyway, or cancel. Nothing is added twice by accident.
Pick it from the dropdown and press + Add to record. It appears as a chip, the patient sees it on their plan, and tapping it any time brings up its goals.
Conditions the patient recorded themselves appear alongside yours, marked 👤.
Open the patient and the goal list shows everything they are working on. Each goal carries a badge: 🩺 you for goals you set, 👤 patient for goals they added themselves. Their check-ins appear under What the patient logged this visit.
A patient cannot edit your target — they ask instead, and the ask arrives on the goal.
✓ marks it achieved: it drops out of the patient’s active list but stays on the record. ✕ removes it entirely. You can do either to any goal, including ones the patient added.
Your practitioner’s goals are at the top, each with its target — Hydration 🎯 64oz/day. Below them, under My goals, are any you have added yourself.
Tap the control under a goal — a slider, a 0–10 scale, or just done, depending on what is being tracked. A ✓ logged badge appears, and your practitioner sees it before your next appointment.
Logging is the whole job. You do not need to write anything unless you want to.
It joins the same list your practitioner sees, marked as yours. You do not need anyone’s approval, and you can change or remove it whenever you like.
When a target does not fit your life, say so — on the goal itself.
Under 🩺 My conditions, add anything you are dealing with — Low back pain, Insomnia. Your practitioner sees it, and each condition offers the goals that usually go with it:
One tap starts tracking it. Goals you already have are not offered again.
| 🩺 Practitioner | 👤 Patient | |
|---|---|---|
| Add a goal | Yes | Yes |
| Log progress | Sees the logs | Yes |
| Change a target the practitioner set | Yes | No — can ask |
| Change a goal the patient added | Yes | Yes |
| Remove a practitioner’s goal | Yes | No |
| Remove their own goal | Yes | Yes |
| Add a condition | Yes | Yes |
A target is a clinical instruction. If it could be changed quietly, the practitioner would be treating from a number they did not set and might never notice. So the patient asks instead, the ask lands on the goal, and nobody has to guess whether it was seen.
